Frequently Asked Questions
Learn more about how Avid Care operates, who we serve, and how to access our services.
What services does Avid Care provide?
We provide Advanced Wound Care, Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and IV Therapy. These services are designed to support patients beyond the traditional medical office.
Who can refer a patient?
Referrals can be made by primary care physicians, specialists, hospital discharge planners, skilled nursing facilities, home health agencies, and patients or their families directly (though a physician order is typically required for clinical services).
Does a patient need a physician order?
Yes. Our clinical services, including advanced wound care, RPM, and IV therapy, require a valid order from a treating provider. We coordinate with the patient's primary care physician to obtain necessary documentation.
Does Avid Care accept insurance?
Coverage depends on the specific service, the patient's insurance plan (e.g., Medicare, Medicaid, commercial), and clinical eligibility. Many of our services, like CCM and RPM, are covered by Medicare for eligible beneficiaries. We verify eligibility prior to initiating care.
How does Remote Patient Monitoring work?
Eligible patients receive a connected device (such as a blood pressure cuff or weight scale). They take their readings at home, and the data is securely transmitted to our clinical dashboard. Our team monitors the data and alerts the primary care provider if readings fall outside of normal ranges.
What conditions may qualify for wound care?
We commonly support diabetic foot ulcers, pressure injuries, venous and arterial leg ulcers, non-healing surgical wounds, and complicated skin tears.
Can Avid Care work with nursing facilities and assisted living communities?
Yes. We frequently partner with SNFs and ALFs to provide on-site wound care rounds or implement RPM programs for residents, coordinating closely with facility nursing staff.
How quickly are referrals reviewed?
Our intake team typically reviews preliminary referrals within 24-48 business hours. Urgent clinical needs should be directed to a local emergency department or hospital.
